サイナスリフト？ソケットリフト？
インプラントの埋入に際してそこに埋入に十分な量の骨がないといけないことは今までこのコーナーでも紹介してきました。
骨量に関して特に問題となることが多いのが上顎の臼歯部（上の奥歯）です。
上顎の臼歯部にインプラントを埋入するときに問題となるのが『上顎洞』という空洞です。これは誰にでもあるのですが、大きさが問題となります。特に加齢により大きくなるとも言われています。また、歯を失うと上顎洞の一番下の面（底の部分）が下がってくるようです。当然空洞ですから、インプラントの維持のためには邪魔な存在です。
あるインプラントの勉強会に参加していたとき、聞いた話ですが、日本人の上顎臼歯部の骨の高さ（深さ）をパノラマ上で計測した平均値が１２mmだそうです。これは歯が残っているときの平均値ですから、当然歯周病あるいはそれ以外の原因で歯を喪失した場合、骨の量は減るわけで、１２mm以上骨の高さが保存されているケースは非常に稀となるわけです。１２mmというのはある意味ボーダーラインな長さで、私たちインプラントを行う立場とすると是非とも確保したい埋入深度量なのです（特に大臼歯部は）。では自然の歯はどうなっているのか？自然の歯はねじのような形をしているわけではありませんから、上手に骨の中に埋まっているわけですね（上顎洞という上あごの空洞を避けて．．）。インプラントの形も天然歯のような形のものができれば、このボーダーももう少しすなくてもすむと思うのですが．．．。
さて、実際骨の量が少ないとき、どうすればいいのか？当然骨の量を増やすしかないわけです。そこで現在使用されているインプラント埋入のテクニックが『サイナスリフト』と『ソケットリフト』です。
＜ソケットリフトについて＞
ソケットリフトは比較的オリジナルの骨の量がある場合に用いられる手法でオリジナルの骨量が１０－１２ｍｍ程度の場合におこなわれます。なぜならこの手法の場合骨を増やす量に限界があるからです。もちろんものすごいテクニックを持ったスーパーデンティストなら限界はないのかもしれませんが（笑）。
このように下の方から骨補填材や自家骨を押し込んでいって骨量をかさ上げします。正直言ってブラインドテクニックなので難しいです。好きな人は好きなんですけどね(^^;)
＜サイナスリフトについて＞
サイナスリフトはさらに骨の量が少ない場合におこなわれる手法で、オリジナルの骨が１０mm以下の場合におこなわれます。さらにインプラントの埋入時期もオリジナルの骨の量が５－６mm以上の場合は同時埋入（インプラントの埋入とサイナスリフトを同時におこなう）をおこなうこと（Simultaneous Approach）もありますし、それ以下（５mm以下）の場合はサイナスリフトをおこなって後骨の造成が確認されてからインプラント埋入をおこなう（Staged Approach）ことがあります。
上顎洞の側壁からアプローチします。
骨ができるとインプラント埋入です。
この方法は外科的には明示下で行えるのでソケットよりは私好きなんですけどね。
いずれにせよこのような方法で上顎の臼歯部の骨の高さを増してからインプラント埋入はおこなわれます。
もちろん、あまり自分の骨をなくしすぎないのが一番ですけどね。
歯を残すことに固執するあまり骨まで失ってしまって後の処置が難しくなるのは考えものではないかと時々思うのです。
