症例紹介 ケース007 骨はあっても角化歯肉がない！！
今回は今までとちょっとテーマが違います。今まではインプラントを埋入するための骨の回復ということが大きなテーマでした。これは大月歯科医院でのインプラント治療の中の大きな柱の一つとなっています。そして今回のテーマはインプラント周囲の組織マネージメントで特に『角化歯肉』ということにスポットライトを当てます。これも大月歯科医院におけるインプラント治療の中の大きな柱の一つです。インプラント治療ではインプラント補綴物の設計も大きな予後を左右する因子の一つですが、インプラントの周りの骨や角化歯肉のマネージメントでも大きく予後を左右します。ですから、大月歯科医院ではこれらのことを十分配慮して治療を進めているため、はっきり言って時間と手間がかかります。ですから今月何本！などと予定を立てては進めない部分もあるのです。そして私自身がすべて行いますので、手にかけられる症例には自ずから限界があり、これらを越えて行うことは治療レベルの低下につながると考えていますので、時々治療をお断りしてしまうこともありますが、その点をご配慮くださいね<(_ _)>。
＜骨はあっても角化歯肉がなく、遊離歯肉移植により角化歯肉を獲得した症例＞
さて、本題に入りましょう！患者様は女性で、右下臼歯部の咬合の回復を主訴として来院されました。
初診時のパノラマです。かなり骨はゆとりがありそうです。抜歯してそのままだったとか。下手な義歯を入れて骨吸収を起こしていなかったのがかえって良かったのでしょうね。しかし、長期歯牙喪失により、角化歯肉はかなり消失していました。
角化歯肉が頬側にはほとんど無い。
とりあえず、通法通りインプラントを埋入しました。もちろんＣＴによる術前診査は行っています。
このようにオリジナルの骨に恵まれているケースにお目にかかれることは稀ですよね。
埋入時のパノラマ写真です。一応オリジナルの骨への埋入であること、そして埋入時の初期固定が良好であったことから、治癒期間は約３ヶ月としました。
さてここからがこのケースでのテーマとなるところです。治癒期間中右下６番のインプラントのヒーリングキャップが露出してしまいましたが、感染は認められず治癒経過は良好であったため、２次手術に踏み切りました。しかし、そのせいもあって頬側に角化歯肉はなく、このままではたとえ上部構造ができたとしても、患者自身のプラークコントロールや、感染への抵抗性が低くなってしまいます。従ってこの場合では遊離歯肉移植により、角化歯肉を獲得することにしました。
２次手術時の写真です。角化歯肉は同側の口蓋歯肉から採取します。
手術後の治癒状況です。上の写真と比べてみてください。もりもり角化歯肉ができています。
こうしてインプラント周囲の角化歯肉を獲得してあげることでインプラント治療の永続性を高めているわけです。
この後、最終補綴物の作成へと続いていきます。
このケースでは角化歯肉の獲得法として『遊離歯肉移植』という方法を使いました。
ただし、ある程度使えそうな角化歯肉がインプラント周囲組織にあるときは、ＡＰＦ（根尖側移動術）により、インプラント周囲の角化歯肉獲得を行います。場合によりけりと言うことですよね(^o^)
