症例紹介 ケース005 ベニアグラフト（骨移植）で骨幅の改善をしたケース
先日ベニアグラフト（骨移植）について解説いたしましたので、今回はその症例を紹介したいと思います。
初診時のパノラマです。右下の臼歯部の咬合改善が主訴でした。
パノラマ上では下歯槽神経までの距離はあるようなのですが、欠損状態を長期に放置していたため幅がとても薄くなっていました。ＧＢＲによる骨幅の改善も検討したのですが、あまりに量が多くなるため断念。
ベニアグラフト法による骨移植を行い、骨幅の改善を図ることとしました。
同側の下顎骨からブロックで骨を移植しました。３本ピンで固定してあるのがわかります。
そして、約５－６ヶ月後にインプラントの埋入手術を行いました。
左側はフラップをあけた状態です。移植骨を固定したピンが認められます。このピンを除去し、移植骨が十分に生着しているのを確認後、肉芽組織等をきれいに除去し（インプラントのオッセオインテグレーションを妨げるおそれがあるから）、直径５ｍｍのインプラントを埋入することができました。この時無理矢理力を入れすぎたドリリング、あるいは埋入時のトルクのかけ方を行わないよう注意する必要があります。

そして、オッセオインテグレーションの期間を充分待ってから、補綴処置へと入っていきます。
今後の課題は廷出している右上７番のマネージメントでしょうね。
