症例紹介 ケース001 その１
＜長期ブリッジケースからのインプラント前の骨造成＞一番よく遭遇するケースです。ブリッジは力学的にもその支台となる歯への負担が大きいのでさらに欠損歯が歯根破折等の原因で増えることになります。しかし、それ以上にインプラント治療を行う上で問題となるのは長い間歯がないために、骨がやせてしまってインプラントを埋入するスペースがなくなってしまっていることです。
その昔、私がインプラント治療を勉強し始めた頃ドイツの『ORALTRONICS社』のインプラントでは『マルチタイプインプラント』という発想で様々な骨幅にあわせたラインナップで対応していましたが、ＧＢＲ等の骨を再生するテクニックが一般的になるとともにその意義は現在薄れてしまっているように思えます。でも当時は画期的な考えだったのです。では現在大月歯科医院ではどのように対応しているのかを紹介いたします。
左下の７番がカリエスで抜歯のケースです。７番もさることながら、ダミー部分の骨吸収が著明なのが上から模型を見るとわかります。あきらめてブリッジをさらに延長するのは避けたいですね。
ダミー部分（赤色）はかなり幅が少ないです。６番ダミー部分は吸収しています。
→そこで７番抜歯後、ダミー部分の骨の再生を行って、インプラント治療に適した状態に周囲組織をととのえることにしました。
約６ヶ月後。明らかに幅が増えています。ここからはじめてインプラントの埋入ポイントが決まります。
次回はセットアップ模型からの外科用ステントの作成へと進みます。
